Services available Now

Eligible Participants (all participants must live in Lake County):

  • Family Caregiver to an older adult (60+) or individual with dementia (any age)

  • Grandparents (55+) serving as the primary caregiver for a child (dependent who is 18 or younger or dependent 19-59 who has a disability)

Services:

  • In-home, facility, or adult day care

  • Support groups

  • Utility assistance

  • Emergency services

  • Home renovation

  • Legal services

  • Caregiver training & education

  • Counseling

  • ADRD Stressbusters course

Bacoa CaregiverGap-Filling Services are provided to complement the care provided by caregivers.

Services include:

  • Emergency response systems
  • Emergency transportation
  • Utility bill assistance
  • Legal services
  • Incontinence supplies
  • Required school or activity fees
  • Clothing, shoes, etc.
  • Home Safety Renovation

Bacoa Caregiver AssistanceAssistant Services help caregivers obtain services and resources that are available within their communities.

Services include:

  • Information & Assistance
  • Caregiver Needs Assessment
  • Referrals
  • Access Assistance
  • Follow-Up
  • Outreach

Bacoa CaregiverRespite Services provide temporary, substitute supports or living arrangements for care receivers.

Services include:

  • In-Home Respite Care
  • Adult or Day Care Services
  • Facility Respite

Bacoa CaregiverTraining & Education Services provide education to caregivers either individually or in a group.

Services include:

  • Self-Care Skills
  • Caregiving Skills
  • Caregiver Training Sessions

Bacoa Support GroupsSupport Group Services provide advice, guidance, and support to caregivers on an ongoing basis.

Support Groups that address topics including (but not limited to):

  • Issues of life
  • Issues of role transitions
  • Interpersonal relationships
  • Anxiety
  • Depression
  • Guardianship
  • Health
  • Nutrition
  • Financial literacy

Bacoa CounselingCounseling Services offer advice, guidance, and coaching to an individual caregiver.

Services include:

  • Counseling Services
  • Life Coaching
  • Purposeful Therapeutic Assistance
Bacoa ADRDEligible Participants (all participants must live in Lake County):

Family Caregiver: Adult that provides unpaid, in-home care to person withAlzheimer’s Disease or other dementias (of any age). Including but not limited to:

  • Alzheimer’s Disease
  • Vascular Dementia
  • Dementia with Lewy Bodies
  • Frontotemporal Dementia
  • Parkinson’s Dementia
  • Normal Pressure Hydrocephalus
  • Creutzfeldt-Jakob Disease

ADRD Services assist caregivers of persons with diagnosed or suspected dementia.

Services include:

  • Care
  • Medical Care and Supplies
  • Environmental and Material Aids
  • Community Access
